Efficacy of meniscopexy using Mitek systems for the treatment of temporomandibular joint disc displacement: a literature review

ABSTRACT

Introduction: Internal derangements of the temporomandibular joint (TMJ) are primarily characterized by pain, anterior displacement of the articular disc, and joint sounds. Meniscopexy using bone anchors is a surgical therapeutic option for the management of these disorders. Among the available systems, the Mitek anchor system is one of the most widely used and reported in the literature; however, there is no review that systematically synthesizes its clinical outcomes.
Objective: To describe the clinical outcomes reported in the literature on meniscopexy using Mitek anchor systems in the treatment of TMJ internal derangements.
Materials and methods: A bibliographic search was conducted in PubMed, Embase, Scopus, and LILACS to identify clinical studies related to the placement of Mitek anchor systems in the TMJ.
Results: Eight studies were included, of which six were observational studies and two were clinical trials. Overall, the studies reported pain reduction, improvement in maximum mouth opening, decrease in joint sounds, and adequate disc/anchor positioning in most cases.
Conclusions: The available evidence suggests that meniscopexy with Mitek anchors may be an effective therapeutic alternative; however, more well-designed clinical studies with larger sample sizes and long-term follow-up are needed to more precisely establish the efficacy and predictability of this system.



RESUMEN

Introducción: Los trastornos internos de la articulación temporomandibular (ATM) se caracterizan principalmente por dolor, desplazamiento anterior del disco articular y ruidos articulares. La meniscopexia mediante anclajes óseos constituye una opción terapéutica quirúrgica para el manejo de estas alteraciones. Entre los sistemas disponibles, el sistema de anclaje Mitek es uno de los más utilizados y reportados en la literatura; sin embargo, no existe una revisión que sintetice de manera sistemática sus resultados clínicos.
Objetivo: Describir los resultados clínicos reportados en la literatura sobre la meniscopexia mediante sistemas de anclaje Mitek en el tratamiento de los trastornos internos de la ATM.
Materiales y métodos: Se realizó una búsqueda bibliográfica en las bases de datos PubMed, Embase, Scopus y LILACS, con el objetivo de identificar estudios clínicos relacionados con la colocación de sistemas de anclaje Mitek en la ATM.
Resultados: Se incluyeron ocho estudios, de los cuales seis corresponden a estudios observacionales y dos a ensayos clínicos. En conjunto, los estudios reportaron reducción del dolor, mejoría de la apertura bucal máxima, disminución de los ruidos articulares y adecuada posición del disco/anclaje en la mayoría de los casos.
Conclusiones: Los resultados disponibles sugieren que la meniscopexia con anclajes Mitek puede ser una alternativa terapéutica eficaz; sin embargo, se requieren más estudios clínicos bien diseñados, con muestras mayores y seguimientos a largo plazo, para establecer con mayor precisión, eficacia y predictibilidad de este sistema.
